R. 512 Phase V, I-95 to 1301h Avenue (Willow St.) <br /> WORK ORDER NO . 3 (Engineering) <br /> FOR <br /> PROFESSIONAL SURVEYING SERVICES AGREEMENT <br /> FOR ANNUAL SURVEYING AND MAPPING SERVICES <br /> WITH <br /> Masteller, Moler & Reed, Inc. <br /> In accordance with Contract No . 0390 <br /> This Work Order No . 3 is in accordance with the existing AGREEMENT dated September 23 , 2003 , <br /> and the extension thereof, between Masteller, Moler & Reed, Inc . (SURVEYOR) and Indian River <br /> County (COUNTY) . This Work Order No . 3 amends the agreement as follows : <br /> SECTION I — PROJECT LIMITS <br /> County Road 512 , Phase V — the SURVEY LIMITS shall begin at the termination of County Road 512 <br /> Phase IV (East of I-95 ) and continue westerly on County Road 512 to 500 ' west of Willow Street ( 130th <br /> Avenue) <br /> SECTION II — SCOPE OF SERVICES <br /> Masteller, Moler & Reed, Inc . (SURVEYOR) shall supply field and office Professional Land Survey <br /> services to complete all task as outlined hereon in this work order No . 3 AND the attached (Exhibit <br /> "A") Task description outline . <br /> Brief: <br /> The scope of services for this Work Order No . 3 , is the completion of all task as described in exhibit "A" <br /> attached hereto, and preparation of a "Topographic" Survey for that portion of County Road 512 , Phase V <br /> as mentioned in section I. The survey will be prepared in accordance with the Chapter 177 & Chapter <br /> 472 Florida Statutes and Chapter 61G17 (Florida Minimum Technical Standards) as set forth for these <br /> types of surveys . <br /> SECTION III - TIME FOR COMPLETION <br /> An 80% submittal shall be delivered to the County for review and comments within 60 calendar days <br /> from Notice To Proceed (NTP) . Final submittal will be delivered to the County within 30 calendar <br /> days of receipt of the 80% submittal comments .
