PERMITTEE: Indian River C-vanty Utilities <br /> 1840 25th Street PERMIT �vJMBER; FL0041637 <br /> Vero Beach, FL 32960 EXPIRATION DATE: July 22, 2004 <br /> IRCUD/WestRegional WWTF <br /> b. If the permittee monitors any contaminant more frequently than required by the permit, using <br /> Department approved test procedures, the results of this monitoring shall be included in the <br /> calculation and reporting of the data submitted in the DMR <br /> c . Calculations for all limitations which require averaging of measurements shall use an arithmetic <br /> mean unless otherwise specified in this permit. <br /> d. Any laboratory test required by this permit for domestic wastewater facilities shall be performed by <br /> a laboratory that has been certified by the Department of Health (DOH) under Chapter 64E1 , <br /> F.A.C., to perform the test. On-site tests for dissolved oxygen, A and total chlorine residual shall <br /> be performed by a laboratory certified to test for those parameters or under the direction of an <br /> operator certified under Chapter 61E12-41 , F.A. C. <br /> e. Under Chapter 62460, F.A.C., sample collection shall be performed by following the protocols <br /> outlined in "DER Standard Operating Procedures for Laboratory Operations and Sample <br /> Collection Activities" (DER-QA-001/92). Alternatively, sample collection maybe performed by <br /> an organization who has an approved Comprehensive Quality Assurance pian (CompQAP) on file <br /> with the Department. The CompQAP shall be approved for collection of samples from the <br /> required matrices and for the required tests. <br /> [62-620. 610(18), 12-2446] <br /> 19. Reports of compliance or noncompliance with, or any progress reports on, interim and final <br /> requirements contained in any compliance schedule detailed elsewhere in this permit shall be submitted <br /> no later than 14 days following each schedule date. [62420. 610(19), 124446] <br /> 20. The permittee shall report to the Department any noncompliance which may endanger health or the <br /> environment. Any information shall be provided orally within 24 hours from the time the permittee <br /> becomes aware of the circumstances. A written submission shall also be provided within five days of <br /> the time the permittee becomes aware of the circumstances. The written submission shall contain: a <br /> description of the noncompliance and its cause; the period of noncompliance including exact dates and <br /> time, and if the noncompliance has not been corrected, the anticipated time it is expected to continue; <br /> and steps taken or planned to reduce, eliminate, and prevent recurrence of the noncompliance. <br /> a. The following shall be included as information which must be reported within 24 hours under this <br /> condition: <br /> 1 . Any unanticipated bypass which causes any reclaimed water or effluent to exceed any permit <br /> limitation or results in an unpermitted discharge, <br /> 2. Any upset which causes any reclaimed water or the effluent to exceed any limitation in the <br /> pest, <br /> 3 . Violation of a maximum daily discharge limitation for any of the pollutants specifically listed <br /> in the permit for such notice, and <br /> 4. Any unauthorized discharge to surface or ground waters . <br /> b. If the oral report has been received within 24 hours, the noncompliance has been corrected, and the <br /> noncompliance did not endanger health or the environment, the Department shall waive the written <br /> report. <br /> [62-620. 610(20), 1244461 <br /> 26 D.E.P: - ORLANDO <br /> EXHIBIT NO. 2 <br />
