EMPLOYEE BENEFIT PLAN CHANGES <br /> AMENDMENT TO THE EMPLOYEE BENEFIT PLAN <br /> SAFECO must be notified of any change to the Employee Benefit Plan . Notices of change must be in <br /> writing and provided to SAFECO prior to the effective date of the change . SAFECO must approve <br /> changes in writing before the benefits provided by the changes will be included as Covered Expenses . <br /> If notice is not received prior to the effective date of the change , SAFECO will determine the date , if any, <br /> the benefits that are provided by this change will be considered Covered Expenses . <br /> If SAFECO does not approve a submitted change , SAFECO will not consider the benefits provided by this <br /> change as Covered Expenses , <br /> Only Covered Expenses for benefits provided by the most current SAFECO approved Employee Benefit <br /> Plan will be considered for reimbursement. <br /> LGC 8814 11 /01 <br />
