Prevent 4 Grant <br /> 04-043 - 10-40-01 -062 <br /> AGREEMENT <br /> THIS AGREEMENT made this 4th day of November, 2003 between the BOARD OF COUNTY <br /> COMMISSIONERS OF INDIAN RIVER COUNTY, FLORIDA, 1840 25TH Street, Vero Beach, <br /> Florida 32960 (the "County") and The SUBSTANCE ABUSE COUNCIL OF INDIAN RIVER <br /> COUNTY, 2501 27th Ave . , Suite A-7, Vero Beach, FL 32960 (the "Contractor") . <br /> I. SCOPE OF SERVICES <br /> A. The Contractor will provide a comprehensive range of substance abuse prevention <br /> and educational activities to Indian River County male and female youth during the period of their <br /> program participation without regard to race, color, creed, national origin, sex, or economic status <br /> pursuant to the Ant-Drug Abuse Act of 1988 and the application submitted by the County to the <br /> State of Florida, Department of Law Enforcement, Office of Criminal Justice Grants, ("grant <br /> agreement") . The Contractor will coordinate with the Court and Office of Probation and Parole and <br /> the Substance Abuse Council of Indian River County to identify adolescents for services . <br /> B . If, after the substance abuse problem assessment is conducted, the Contractor <br /> determines that the individual meets program eligibility requirements , the Contractor will make <br /> available to the individual the following range of services : <br /> 1 . Alternative drug free events <br /> 2 . Substance abuse prevention education classes <br /> 3 . Lifeskills development education classes <br /> 4 . Development of a media package <br /> Co The Contractor shall perform and maintain records consistent with the grant <br /> agreement. Said records shall be made available to the County upon request . <br /> D . The Contractor is responsible and accountable for complying with Section G, <br /> Acceptance and Agreement, under the Drug Control System Improvement Formula Grant Program <br /> for FY 2004 Funding. <br /> 1 <br />
