AMENDMENT NO , 2 TO WORK AUTHORIZATION DIRECTIVE 2007 -005 <br /> AND CONTRACT EXTENSION AGREEMENT <br /> This Amendment No . 1 to Work Authorization Directive 2007-005 and Contract <br /> Extension Agreement ( "Amendment") is entered into this 15th day of May 2009 , nunc pro tunc <br /> to May 15 , 2009 . pursuant to that certain Standard Form Agreement Between Owner and <br /> Contractor on the Basis of a Stipulated Price entered into as of February 6 , 2007 <br /> ("Amendment" ) by and between Indian River County , a political subdivision of the State of <br /> Florida ( County "' ) and Underground Utilities , Inc . ("Contractor" ) . <br /> 1 The COUNTY has selected the Contractor to perform work as set forth in Work <br /> Authorization Directive 2007 - 005 dated as of June 19 , 2007 , <br /> 2 The COUNTY and the Contractor desire to amend Work Authorization Directive <br /> 2007-005 by increasing the amount for labor by $25 , 697 .40 for a total amount of <br /> $ 747 , 432 40 , all in accordance with the terms and provisions set forth on the Agreement . <br /> 3 .
