� �tvE,p Board of County Commissioners <br /> O 2 <br /> 1840 25th Street <br /> Vero Beach, Florida 32960 <br /> iF �t Telephone: (772) 567-8000 <br /> �GOR10Q' FAX: 772- 778-9391 <br /> WORK ORDER NO, 19 <br /> "Lift Station 58 Survey" <br /> FOR <br /> PROFESSIONAL SURVEYING SERVICES AGREEMENT <br /> FOR ANNUAL SURVEYING AND MAPPING SERVICES CONTRACT NO : 0390 <br /> WITH <br /> Masteller, Moler, Reed & Taylor, Inc, <br /> This Work Order No. 19 is in accordance with the existing AGREEMENT dated September 19th, <br /> 2006 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County <br /> (COUNTY). This Work Order No. 19 amends the agreement as follows : <br /> SECTION 1 — PROJECT LIMITS <br /> Perform Boundary and Topographic Survey for Lift Station 58 on easement portion of property <br /> located in Indian River County and lying in Section 26, Township 032 South, Range 039 East and <br /> identified as Tax Parcel 32392600000700000001 .4. <br /> SECTION II - SCOPE OF SERVICES <br /> As agreed upon between Masteller, Moler, Reed & Taylor, Inc. and Indian River County, Masteller, <br /> Moler, Reed & Taylor, Inc. will perform Professional Land Surveying services to include the <br /> following : <br /> a. Perform limited boundary survey of Healthsouth property and identify location of existing <br /> Utility Easement per Bill of Sale recorded in Official Record Book 762, Page 350. Boundary survey <br /> shall be prepared in accordance with the Minimum Technical Standards for surveying as adopted by <br /> the State of Florida. <br /> b. Perform Bench Run using NGVD 1929 elevations to the site and establish benchmarks on site. <br /> c . Obtain elevations on site and 25 feet outside of the perimeter of the easement, sufficient to <br /> generate one foot contours. <br /> d . Locate and obtain elevations of all visible improvements on and adjacent to the site. (i. e . : <br /> utilities, culverts, pavement, mailboxes, etc.) <br /> e. Measure and dimensions existing improvements, including wet well, valve box, generator etc . <br /> f. Prepare legal description and sketch of additional easement to be obtained from property owner. <br /> g. Provide five copies and one digital file of the boundary and topographic survey upon <br /> completion. <br /> SECTION III — WORK PRODUCT <br /> a) The SURVEYOR shall prepare its Work Product so that the COUNTY or other consultants <br /> can readily use it for the design and analysis of the area, as defined. It shall contain all <br /> information necessary for third-parry surveyor to independently recreate and/or utilize the <br /> survey work. It is acknowledged all final products become property of Indian River County <br /> and will be available for use by the public at large . <br />
