o <br /> CLARIFICATIONS / CORRECTIONS : <br /> 1 ) The 6, 500 cubic yards of the excavated material from Pond 200 originally proposed to be placed on the <br /> Indian River Federal Credit Union site and the 10 ,000 cubic yards of the excavated material from Pond <br /> 200 originally proposed to be placed on the Elwill Associates, Inc. site shall be amended to not be <br /> placed on the two above sites. <br /> This material is hereby amended to be included in Line Item 120- 1 , EXCAVATION REGULAR. <br /> 2) IRC Utility services : <br /> a) Any service under pavement shall be placed in larger sized conduit sleeve . Any service extensions <br /> shall be constructed by replacing the entire polyethylene service from the water main to the new <br /> meter location. (No splicing permitted). The cost shall be included in Line Item 1645 - 800 WATER <br /> METER (RELOCATE) <br /> b) Prior to construction all valves shall be located by GPS and properly mapped out. The cost shall be <br /> included in Line Item 108- 1 AS -BUILTS <br /> c) All valves shall be maintained and accessible during construction. <br /> d) As-builts of the utilities shall be completed and approved prior to any testing (pressure , <br /> bacteriological , etc.) . The cost shall be included in Line Item Line Item 108 - 1 AS -BUILTS <br /> e) Pressure and/or bacteriological testing shall be included in all utility stub outs . <br /> f) The cost for the sampling points on the utility lines shall be included in the cost of the pipe. Also <br /> included in this cost and the responsibility of the contractor is the jumper for filling, pressure test, <br /> chlorination, filling, flushing, bacteriological testing, etc. <br /> 3 ) All phases of the Maintenance of Traffic (MOT) plan shall be submitted for review and approval by the <br /> Engineering Division. The MOT does not have to be certified by an engineer. <br /> 4) BRIDGE MODIFICATION — The existing pedestrian pole and signal box located at he northwest <br /> corner of the bridge will be re-located by the county prior to construction. <br /> 5) All planted trees and other plantings shall be guaranteed for 120 days after planting . The contractor <br /> must take due care to assure the plantings survival . <br /> 6) Line Item 630- 1 - 12 CONDUIT (F&l) (UNDERGROUND) (2" PVC) shall include two — two inch (2") <br /> conduits to be placed in the same trench. The price per linear foot includes the two conduits. <br /> 7) At approximately Station 194+50 , a bus turnout is shown on the plans (see Sheet 18 ) . At that location, <br /> south of the proposed 8 foot wide sidewalk a 6 . 5 foot by 15 foot — 8 inch thick concrete pad is shown. <br /> The Bus Shelter shall not be included in the contract. <br /> 8) The contract will not require the contractor to pay rent for the use of the Cumberland Farms building <br /> (site office) .
