En.fineering Testing Services <br /> 12t Street and 27th Avenue Improvements — IRC Project #9334 <br /> Indian River County, Florida <br /> Dear Mr. Weragoda: <br /> INTRODUCTION <br /> As requested, Dunkelberger Engineering & Testing, Inc. (Dunkelberger) is pleased to submit this <br /> proposal for Engineering Testing Services during construction activities at the subject project in Vero <br /> Beach (Indian River County), Florida. <br /> PROJECT INFORMATION <br /> The project involves drainage and roadway improvements to approximately 'h mile of 12th Street <br /> starting about 250 feet east of 32'd Avenue through the intersection at 27th Avenue to about 400 feet <br /> east of 25th Avenue . The proposed drainage improvements primarily involve the installation of about <br /> 2,400 lineal feet (If) of 60-inch diameter reinforced concrete pipe (RCP) and 1 ,781 if of 8-inch <br /> diameter Type II underdrain. Associated with the primary drainage trunk line will be secondary <br /> drainage consisting of 124nch, 15-inch and 184nch diameter RCP. The roadway improvements will <br /> include new construction (road widening) and asphalt milling. In addition, there will be concrete <br /> headwalls, sidewalks and driveway construction. Also, a traffic signal mast arm will be installed at the <br /> intersection of 12th Street and 27th Avenue. <br /> The scope of work described in this proposal is based on our review of the construction drawings <br /> prepared by Indian River County — Engineering Division, dated May 25 , 2012, and our experience <br /> with similar projects for you in the past. <br /> SCOPE OF SERVICES <br /> We will provide the required construction materials testing services for the project.
