The final Request for Funds <br /> must be submitted with the closeout since funds not requested will be deobligated at closeout . <br /> Any <br /> amendment needed to close out the contract should be submitted prior to submission of a closeout . <br /> Closeout requirements can be found in Rule 9B - 43 . 0051 ( 5 ) , F . A . C . <br /> 1 . All grant recipients must complete the following sections : <br /> SECTION A - CONTRACT INFORMATION <br /> SECTION E - STATUS OF ACCOMPLISHMENTS AND EXPENDITURES <br /> SECTION F - BENEFICIARY DATA <br /> SECTION I - CLOSEOUT APPROVAL <br /> Section D relates to housing activities . <br /> Section H relates only to disaster recovery program grants . <br /> 4 . Additional copies of forms may be made if there is insufficient space to enter the information <br /> that needs to be reported . <br /> 5 .
