Attachment #1 <br /> Amendment to Work Order Number 4 <br /> Additional Permitting Services for <br /> Algal Turf Scrubber System at <br /> 5`h Street SW Site <br /> South RO Plant Concentrate Disposal <br /> PROJECT DESCRIPTION <br /> This Amendment to Work Order #4 includes additional permitting and design services as <br /> requested by the Indian River County Utility Department, beyond what was included in <br /> Work Order #4 and the previous Work Order #2 . In summary, the additional scope of <br /> work in general, consists of. <br /> IRFWCD Permitting <br /> The County has requested that Kimley-Horn and Associates prepare and submit the permit <br /> applications to the Indian River Farms Water Control District Board (IRFWCD) for the <br /> proposed improvements at the Algal Turf Scrubber (ATS) site . The permit applications <br /> will be for the proposed driveway connection and culvert, the removal of the existing <br /> driveway and culvert, and ancillary improvements to the Sub- lateral J- 1 canal. <br /> Additional Environmental Resource Permitting <br /> The County has requested the Kimley-Horn and Associates prepare and submit a Standard <br /> General Environmental Resource Permit (ERP) application and permit documents to the <br /> Florida Department of Environmental Protection (FDEP) and the United States Army <br /> Corps of Engineers (USACE) for the proposed stormwater pump station, the ATS site, the <br /> ATS stormwater pipe, the access road from the adjacent South County Water Treatment <br /> Plant (WTP) to the ATS site, and ancillary improvements to the Sub- lateral J- 1 canal. As <br /> part of Work Order #4, Kimley-Horn was responsible for permit documents related to the <br /> stormwater pipe only; however, the County has since requested that Kimley-Horn and <br /> Associates prepare and submit the application and supporting documents for all of the ATS <br /> project. <br /> • This work order includes additional permitting services to prepare the Standard <br /> General ERP application and supporting documents for the stormwater pump <br /> station, ATS site, the ATS stormwater pipe, the access road from the adjacent <br /> South WTP to the ATS site and ancillary improvements to the Sub- lateral J- 1 <br /> canal.
