J <br /> AMENDMENT NO , 3 TO THE PROFESSIONAL CIVIL ENGINEERING i <br /> SERVICES AGREEMENT FOR CIVIL ENGINEERING , SURVEYING <br /> AND BRIDGE DESIGN SERVICES <br /> 41ST BRIDGE REPLACEMENT OVER IRFWCD LATERAL A CANAL <br /> (66TH AVENUE) <br /> AMENDMENT NO. 3 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING <br /> SERVICES AGREEMENT BETWEEN KIMLEY-HORN AND ASSOCIATES , INC. AND <br /> INDIAN RIVER COUNTY, FLORIDA. <br /> This is an amendment to the existing Engineering/Land Surveying Services Agreement <br /> (AGREEMENT) dated April 1St, 2003 between Kimley-Horn and Associates, Inc . (ENGINEER) <br /> and Indian River County (COUNTY) . This amendment addresses changes in " Section III - <br /> Scope of Services " , " Section V - Compensation " and " Section XVI — Duration of Agreement " of <br /> the AGREEMENT . <br /> Amendment Description <br /> This Amendment includes the following : <br /> " SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following : <br /> Utility Design <br /> The ENGINEER will develop construction documents to detail the extension of the <br /> existing 12 " water main, located along 41St Street east of 66th Avenue , over the proposed <br /> bridge to be constructed on 41 st Street to the western right of way along 66th Avenue . In <br /> addition, the existing 12 " reuse main will be modified to provide for a service connection <br /> along the western right of way along 66th Avenue . <br /> Utility Permitting <br /> The ENGINEER will incorporate these utility modifications within a FDEP public <br /> drinking water permit and/or notification as required . The consultant will coordinate <br /> with FDEP in regards to obtaining these permits and/or authorizations . The COUNTY <br /> will be responsible for all permit application and agency review fees . <br /> Stormwater Permitting <br /> The ENGINEER will submit a Standard General Enviromental Resource Permit to St . <br /> Johns River Water Management District to address the proposed roadway improvements <br /> along 41 st Street that is associated with the 66th Avenue/ 41 st Street Intersection <br /> Improvement . The proposed 66th Avenue roadway and drainage improvements is <br /> intended to be permitted by others . The COUNTY will be responsible for coordinating <br /> with Indian River Farms Water Control District regarding the time extension of the <br />
