SUPPLEMENTAL CONTRACT NO . <br /> 80- 939-7102 AGREEMENT AQ -822 <br /> The Florida Department of Transportation and Indian River County desires to supplement the <br /> original Agreement entered into and executed on March 25th , 2011 as identified above . All provisions in the original <br /> Agreement and supplements , if any, remain in effect except as expressly modified by this supplement. <br /> The changes to the Agreement and supplements, if any , are described as follows: <br /> PROJECT DESCRIPTION <br /> Name Sidewalk Construction on Old Dixie Highway from 38th Lane to 65th Street with a pedestrian bridge over the North Relief <br /> Canal <br /> Length 3 . 496 Miles <br /> Termini Old Dixie Highway from 38th Lane to 65th Street <br /> Description of Work: <br /> Sidewalk Construction on Old Dixie Highway from 38" Lane to 65" Street with a pedestrian bridge over the North Relief Canal <br />. <br /> Reason for Supplement and supporting engineering and/or cost analysis : <br /> As a result of unexpected right-of way issues , 20% of the project in the southern limit was eliminated from the scope of work <br />. The <br /> revised project limit starts at 45th Street and ends at 65"' Street. A revised scope with the decrease in quantities was submitted <br /> by the <br /> Contractor, Goodson Paving Inc. , to the County. <br /> The terms of the original Agreement, referenced above, are hereby amended as follows: <br /> - This Supplemental Agreement decreases the Total Federal Funds in the Original Agreement by $438 , 324. 26 , as explained <br /> below. <br /> Total Revised Bid Amount $ 1 , 074 , 121 . 26 <br /> Non- Participating Items ($63 , 517 . 92 ) <br /> Total Participating Items $ 1 , 010 , 603 . 34 <br /> CEI Costs $ 121 , 272 . 40 <br /> Total Project Costs ( Participating Items + CEI ) $ 1 , 131 , 875 . 74 <br /> - Paragraph 3 . 01 of the Original Agreement is hereby amended to adjust the total cost of the project to $ 1 , 131 , 875. 74 <br /> - Exhibit 1 included in the Original Agreement is hereby amended to adjust the Federal Funds to $ 1 , 131 , 875 . 74 <br /> - See Exhibit B attached hereto and made a part of hereof which replaces Exhibit B attached to the original Agreement. <br /> - This Supplemental Agreement also amends the Original Agreement to include Paragraph 13. 16 E-VERIFY as follows : <br /> 13 . 16 E-VERIFY <br /> The Agency : <br /> 1 . Shall utilize the U . S . Department of Homeland Security's E-Verify system to verify the employment eligibility of all new <br /> employees hired by the Agency during the term of the contract, and <br /> 2 . Shall expressly require any subcontractors performing work or providing services pursuant to the state contract to likewise <br /> utilize the U . S . Department of Homeland Security 's E-Verify system to verify the employment eligibility of all new employees <br /> hired by the subcontractor during the contract term . <br />
