Number of years the firm has performed business as a Contractor in work of the <br /> type involved in this contract : 20+ years <br /> 5 . What is firm 's bonding capacity? $ 40 Million <br /> 6 . Attach to this questionnaire , a notarized financial statement and other information <br /> that documents the firm 's financial strength and history . <br /> 7 . Attach a list of at least three (3 ) separate and verifiable clients . At least on of the <br /> client references shall be for a disaster recovery project . <br /> 8 . Attach a detailed description of the Contractor' s organizational structure , "chain of <br /> command " of Contractor's response team and project management methods . <br /> Provide the name and address of Subcontractors and detail the intended scope of <br /> their work . <br /> 9 . Attach a list of types and quantities of recovery equipment , availability of <br /> equipment , estimate of workload , and future commitments to other emergency <br /> response contracts .
