Sector 3 Beach & Dune Restoration Project Attachment 1 to Chancre Order No . 1 All reports or notices relating to this permit shall be emailed and sent to the Department at the following locations : DEP Bureau of Beaches & Coastal Systems JCP Compliance Officer Mail Station 300 3900 Commonwealth Boulevard Tallahassee , Florida 32399 - 3000 phone : ( 850 ) 414 -7716 e - mail : JCP Compliance a ,dep . state . fl . us Item 25 : Technical Specifications — 6 . 5 Acceptance Sections and Pay Profiles Section 6 . 5 is clarified as follows : The following_procedure shall be implemented to coordinate pay survey The CONTRACTOR will contact the COUNTY ' s inspector via phone or in person prior to 3 . 00 PM to identify the stations which the CONTRACTOR considers ready for a post- fill pay survey . The COUNTY ' s infector will directly convey the stations to COUNTY staff (James Gray) and the ENGINEER ' s representative (Tem Fontaine) via email . Following review by the ENGINEER of observations by the COUNTY ' s inspector and/or the ENGINEER' s representative — indicating, fill placement is complete for the stations , the ENGINEER will initiate performance of the pay surveys by the SURVEYOR with corresponding notification to the CONTRACTOR and COUNTY via email . The above is intended to : ■ avoid duplicate pay profile surveys , and ■ better enable the COUNTY and the ENGINEER to keep track of the CONTRACTOR' s fill operations . Item 26 : Technical Specifications — 6 . 4 Placement & Grading Section 6 . 4 is amended as follows : 6 . 4 . 8 Templates : To better enable the CONTRACTOR to control and shape the fill template in the field to avoid adverse impacts to existing vegetation and or structures the following; measures shall apply : ( 1 ) For each "pay profile" the SURVEYOR will prescribe the template as a linear interpolation of the templates prescribed on the Drawings for the surrounding FDEP profiles . (2) The SURVEYOR will not adjust the interpolated template at the landward or seaward ends of the ` pay profile" templates ; instead adjustments at the seaward and landward ends of the "pay profiles" may be made in the field subject approval by the COUNTY ' s inspector and in accordance with the following : (a) At the seaward end the "pay profile" template will extended seaward of the beach berm at a l OH : I V slope to match the existing; grade . (b) At the landward end the "pay profile" template will either : ■ extend to intersect the existing beach contour if the existing vegetation line is at or above the proposed dune crest or above the berm crest where no dune is proposed, Page 25 of 26 March 16, 2010
