QUALIFICATIONS QUESTIONNAIRE <br />Answer to question # 8 Section 00456 OSHA Violations <br />Job Location: Hypoluxo Road, West Palm Beach, FL <br />Owner: Palm Beach County <br />Owner's telephone #: 561-684-4180 <br />On April 24, 2009, Ranger Construction Industries, Inc. was cited under subpart 1926.701(b) <br />Concrete and Masonry Construction• Reinforcing steel All protruding reinforcing steel, onto <br />and into which employees could fall, shall be guarded to eliminate the hazard of impalement. <br />Circumstances: <br />A gap was present in a barrier wall where rebar protruded from the walls edge into the gap. The <br />gap was very small and would prove to be difficult for a person to attempt to use the area as a <br />means of passing from one side of the wall to the other. OSHA asserted that an employee could <br />choose to attempt to use the gap as a walkway, and receive lacerations or abrasions from the <br />rebar The condition was quickly abated.
