525-010-40 <br />CONSTRUCTION <br />OGC-11/03 <br />FPN NO 409504-1-38/58-01 <br />EXHIBIT A <br />Project Description and Responsibilities <br />This exhibit forms an integral part of that certain Reimbursement Agreement between the State of Florida, Department of <br />Transportation and <br />Indian River County <br />Dated 06/02/2005 <br />PROJECT LOCATION: <br />Indian River Drive, Sebastian, FL <br />This project 0 is ® is not on the National Highway System. <br />This project 0 is is not. on the State Highway System. <br />PROJECT DESCRIPTION: <br />Construct an 8' wide concrete bike path along the east side of Indian River Drive between the north Sebastian City limits <br />to Roseland Road. <br />SPECIAL CONSIDERATION BY AGENCY: <br />None <br />SPECIAL CONSIDERATION BY DEPARTMENT: <br />Funding for the design portion is in the year 2005/2006 in the amount of $15,000.00. Funding for the construction portion <br />is in the year 2006/2007 in the amount of $247,000.00 for a total of $262,000.00. Upon execution of the contract by all <br />parties the department will issue a "Notice to Proceed" for the design portion only. DO NOT START ANY DESIGN <br />UNTIL THE AGENCY RECEIVES A FULLY EXECUTED AGREEMENT WITH A "NOTICE TO PROCEED." Funds for <br />the construction portion will not be available until after July 1, 2006. A second "Notice to Proceed" for construction will <br />not be issued by the department to the agency until the design portion is final billed and the department has approved the <br />design plans, the advertisement, the bid package, the specifications, and scope. THE AGENCY SHOULD NOT START <br />ANY CONSTRUCTION UNTIL THE DEPARTMENT ISSUES THE SECOND "NOTICE TO PROCEED." The <br />department will only reimburse for costs from the execution date (all parties) to the expiration date of the contract. Any <br />costs incurred by the agency prior to the execution date or after the expiration date will not be entertained by the <br />department. <br />
