PaILIC DONOR <br />August 23, 1975 <br />r DISTRICT IX <br />State of Florida <br />Department of Health and Rehabilitative Services <br />AGREEI-EENT <br />M <br />THIS AGREEMENT, entered into this 20th day of September 19 76, <br />by ind between the State of Florida, Department of Health aiid :;ehabi=ita�ive <br />Services, hereinafter referred to as the "Department", and Indian I;.iver County Board <br />of Count, Cornissioners — <br />hereinafter referred to as the "Donor", <br />WIT`ESSETH: , <br />WHEREAS, the State of Florida, Department of Health and Rehabilitative <br />Services'by authority granted in Chapters 20 and 409, Florida Statutes, 1975, <br />administers federal funds under Title XX of the Social Security Act which must <br />be matched with state and/or local funds in accordance with Title 45, Code of <br />Federal Regulations, Chapter II, Part 228; or if said provision is revoked, <br />revised, and/or transferred to a new part in the Code of Federal Regulations, <br />said funds m,-,st be matched in accordance with the provision of the new applicable <br />part of the Federal Regulations; and all amendments thereof; and, <br />WHIEREAS, Donor wishes to make a financial contribution to t o Depar, .ent <br />to be used :or the provision of services, said contribution of the Donor. to be <br />matched by federal funds; and, <br />WHEREAS, services will be provided to those eligible to receive said <br />services under the standards of eligibility of the Department, - <br />N -M, THEREtORE, in consideration of the mutual undertakings and <br />agreements hereinafter set forth, the Department and the Donor agree as foilows: <br />1. Donation j <br />A. Donor shall contribute to the Department the sL of $2,325.00 cash J <br />in cash for the purpose of obtaining state fu:ds <br />in the amount of $ •,433. 0 and federal funds in the amount of $17,285.25 <br />for a total c•f $23�04Fi.25 to be used for the provision of IjTrans.,Field I <br />services to those eligible under the standards of eligibility of the Department. <br />HKA <br />Donor's contrib:tion shall be made in one _ payof $2,325.00 <br />with the first of said payment being made on November ] 1276 _,'and subsequent <br />o <br />pa•—.-ents being nade on none <br />B. Certified public e::penditures may vary from contribution to <br />contribution but the total amount of the certified public expenditures at the end <br />of this contract p_riod will not <br />be more than the agreed-upon total contributior <br />due under this contract.
