STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION <br />LOCAL AGENCY PROGRAM AGREEMENT <br />525-010-40 <br />PROJECT MANAGEMENT OFICBUDGET <br />06/06 <br />Page 13 <br />FPN: 416083-1-58-01 <br />EXHIBIT "A" <br />Project Description and Responsibilities <br />This exhibit forms an integral part of the Agreement between the State of Florida, Department of Transportation and <br />Indian River County <br />Dated <br />PROJECT LOCATION: The project is located in Sections 11 and 12, Township 33S, Range 39E along the North side of <br />12th Street between Vero Beach Elementary and 11th Avenue in Indian River County, Florida. <br />The project is X is not on the National Highway System. <br />The project <br />is <br />X <br />is not on the State Highway System <br />PROJECT DESCRIPTION: 12th Street sidewalk, Vero Beach Elementary to 11th Avenue. The Project consists of an 6' <br />wide concrete sidewalk along the north Right -of -Way of 12th Street. The project length is approximately 0.4 miles. The <br />existing swale drainage along the north R.O.W. will need to be relocated to an area between the existing road and the <br />proposed sidewalk or piped in some locations. The proposed sidewalk will connect to the existing 6' wide sidewalks <br />located at Vero Beach Elementary and at 11th Avenue. <br />SPECIAL CONSIDERATION BY AGENCY: <br />The audit report(s) required in the Agreement shall include a Schedule of Project assistance that will reflect the <br />Department's contract number, the. Financial Project Number (FPN), the Federal Authorization Number (FAN), <br />where applicable, the amount of State funding action (receipt and disbursement of funds), any Federal or local <br />funding action and the funding action from any other source with respect to the project. <br />The Agency is required to provide a copy of the design plans for the Department's review and approval to <br />coordinate permitting with the Department, and notify the Department prior to commencement of any right-of- <br />way activities. <br />The Agency shall commence the project's activities subsequent to the execution of this Agreement and shall <br />perform in accordance with the following schedule: <br />a) Study to be completed by . (Phase 18 and 28 LAP Agreements) <br />b) Design to be completed on or before . (Phase 38 LAP Agreements) <br />c) Right -of -Way requirements identified and provided to the Department by . (All LAPs <br />requiring R/W) (District will handle all Right -of -Way activities on LAPs; this date would be set by the <br />necessary timeframe to complete RNV activities) <br />d) Right -of -Way to be Certified prior to advertising for Construction. (All Phase 58 LAPS) <br />e) Construction contract on be let on or before (For Phase 58 LAPs) (This date would be prior <br />to the end of the Fiscal Year that the Phase 58 is programmed in FM) <br />f) Construction to be completed on or before .
