STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION LOCAL AGENCY PROGRAM AGREEMENT 525-010-40 PROJECT MANAGEMENT OFICBUDGET 06/06 Page 13 FPN: 416083-1-58-01 EXHIBIT "A" Project Description and Responsibilities This exhibit forms an integral part of the Agreement between the State of Florida, Department of Transportation and Indian River County Dated PROJECT LOCATION: The project is located in Sections 11 and 12, Township 33S, Range 39E along the North side of 12th Street between Vero Beach Elementary and 11th Avenue in Indian River County, Florida. The project is X is not on the National Highway System. The project is X is not on the State Highway System PROJECT DESCRIPTION: 12th Street sidewalk, Vero Beach Elementary to 11th Avenue. The Project consists of an 6' wide concrete sidewalk along the north Right -of -Way of 12th Street. The project length is approximately 0.4 miles. The existing swale drainage along the north R.O.W. will need to be relocated to an area between the existing road and the proposed sidewalk or piped in some locations. The proposed sidewalk will connect to the existing 6' wide sidewalks located at Vero Beach Elementary and at 11th Avenue. SPECIAL CONSIDERATION BY AGENCY: The audit report(s) required in the Agreement shall include a Schedule of Project assistance that will reflect the Department's contract number, the. Financial Project Number (FPN), the Federal Authorization Number (FAN), where applicable, the amount of State funding action (receipt and disbursement of funds), any Federal or local funding action and the funding action from any other source with respect to the project. The Agency is required to provide a copy of the design plans for the Department's review and approval to coordinate permitting with the Department, and notify the Department prior to commencement of any right-of- way activities. The Agency shall commence the project's activities subsequent to the execution of this Agreement and shall perform in accordance with the following schedule: a) Study to be completed by . (Phase 18 and 28 LAP Agreements) b) Design to be completed on or before . (Phase 38 LAP Agreements) c) Right -of -Way requirements identified and provided to the Department by . (All LAPs requiring R/W) (District will handle all Right -of -Way activities on LAPs; this date would be set by the necessary timeframe to complete RNV activities) d) Right -of -Way to be Certified prior to advertising for Construction. (All Phase 58 LAPS) e) Construction contract on be let on or before (For Phase 58 LAPs) (This date would be prior to the end of the Fiscal Year that the Phase 58 is programmed in FM) f) Construction to be completed on or before . (Phase 58 LAP Agreements) If this schedule cannot be met, the Agency will notify the Department in writing with a revised schedule or the project is subject to the withdrawal of federal funding.
