Collins, Jr. <br />Re: John McIntosh #13521298007 <br />Dear Mr. Collins: <br />Your letter of 5/2/78 to Mr. Lombardo has been referred to me for response, <br />my apologies for the delay. <br />I have reviewed Mr. McIntosh's account, I think the following are key issues: <br />1. Patient was an emergency transfer from Indian River Memorial Hospital. <br />The attached medical records and correspondence state that the patient <br />is "not treatable in Indian River County". <br />2. The patient was admitted on 1/7/78 (Saturday), on 1/10/78 we advised <br />Indian River County of the patient's admission via the attached farm <br />letter. <br />3. On Jan. 27, 1978, Indian River County wrote J.M.H. a letter accepting <br />ISI. <br />responsibility and asked for additional clerical information. On 3/3/78 <br />we forwarded the name and address of the patient's mother to Ms. Janice <br />Lindsey. <br />Please note the patient's referral on 1/7/78 and Ms. Lindsey's letter. <br />..of 1/27/78. There is not a mention of twelve (12) days. <br />4. On 4/12/78, we billed Indian River for $16,456.00 (68 x $242.00). <br />5. On 4/19/78 Ms. Lindsey wrote J.M.H. taking exception to our biliing. <br />. 6.
