ON MOTION by Commissioner Adams, SECONDED by <br />Commissioner Lowther, the Board unanimously approved <br />Renewal of the Class “B” Certificate of Public <br />Convenience and Necessity for Indian River Memorial <br />Hospital, to be effective for a period of two (2) years from <br />November 19, 2004, to November 19, 2006, as <br />recommended in the memorandum of October 29, 2004. <br /> <br />CERTIFICATE IS ON FILE IN THE <br />OFFICE OF THE CLERK TO THE BOARD <br /> <br />11.C. CITRUS BEST MANAGEMENT PRACTICES (BMP) <br />CONTRACT EXTENSION - FLORIDA DEPARTMENT OF <br />AGRICULTURE AND CONSUMER SERVICES (FDACS) <br /> <br />ON MOTION by Commissioner Adams, SECONDED by <br />Commissioner Neuberger, the Board unanimously <br />authorized (A) extending the time for the contract deadlines <br />on listed applicants until March 1, 2005: #338, #313, #306, <br />#322, #322C, #322B, #307, #305, #327, #309, #308,.#330, <br />#234, #334, #310; (B) delegating to the County <br />Administrator or his designee the authority to sign letter <br />agreements to each applicant extending the contract <br />deadline to March 1, 2005 on a form prepared and approved <br />by the County Attorney’s Office, as recommended in the <br />memorandum of November 1, 2004. <br /> <br /> November 9, 2004 <br />19 <br /> <br />
