OCT 10 1979 OK 4 1 `'AGUE69 <br />Board of County Commissioners <br />Indian River County _ <br />N <br />SOUTHEMI BELL TELEPHOTTE AIND TELEGRAPH COMPAiTY <br />LETTER OF ELECTION <br />The undersigned Subscriber requests the Southern Bell Telephone and Tele-. <br />graph Company to provide, at the address designated below, equipment in accordance <br />with the Telephone Company's lawfully filed tariffs. Important tariff provisions <br />relating to Two -Tier pricing are set forth herein: <br />1. Southern Bell will provide, install, maintain and provide mainte- <br />nance service for the following basic equipment and any subsequent <br />additions subject to the availability of parts: <br />Dimension 400-76 <br />2. The subscriber agrees to pay Southern Bell for the equipment <br />described -above. The subscriber will pay: <br />(Check appropriate box) <br />A One Time Payment price or <br />—) A Tier A monthly price for -60 months, _ <br />the Initial Service Period <br />plus the Tier B monthly price for the duration of service. In -the case of Alter- <br />nate Plan 1, an installation charge also applies. Tier A monthly prices are not <br />subject to change during the Initial Service Period. Tier B prices are subject <br />to chance at any time in conformity with currently effective tariffs. <br />In the event of disconnection of all or part of the equipment prior to <br />the expiration of the Tier A Initial Service Period, the Subscriber will be re- <br />quired to pay a termination charge equal to the present worth of the remaining <br />Tier A payments at the interest rate used in determining the Tier A monthly <br />rates. A lump sum payment for the Tier A balance due will be made. Considera- - <br />tion will be given as to the reusability of the equipment and credit given the <br />Subscriber, if appropriate. <br />At any time during the Tier A Initial Service Period the Subscriber has the <br />option of making a lump sum payment of the Tier A balance using -the procedures <br />described for disconnection of equipment. <br />
