COMMUNITY SERVICES TRUST FUND GRANT AGREEMENT <br />THIS AGREEMENT, beginning the lst day of OCTOBER <br />19 79 , between the State of Florida Department of Community <br />E <br />Affairs (hereinafter referred to as "Department"), an agency of <br />the State of Florida, with headquarters in the City of Tallahassee, <br />and INDIAN RIVER COUNTY COMMISSION <br />(hereinafter referred to as the "Grantee"). <br />THIS AGREEMENT (CONTRACT) IS ENTERED INTO BASED ON THE FOLLOWING <br />FACTS: <br />The Department, in furtherance of its duties under <br />Sections 409.501 through 409.506, Florida Statutes has determined <br />that the Grantee has applied and qualifies for a grant under the <br />Florida Financial Assistance for Community Services Act. <br />NOW, THEREFORE, THE PARTIES HERETO DO MUTUALLY AGREE AS FOLLOWS: <br />1. Services. <br />Grantee will provide the services described in the <br />attached work program(s), by this reference made a part of <br />this agreement. <br />2. Incorporation of Rules. <br />Both, Grantee and Department will be governed by <br />applicable law and rules, including but not limited to <br />§§409.501--.506, Florida Statutes and Rule 9C-2, Florida <br />Administrative Code. <br />3. Compensation. <br />a) The Department agrees to pay the Grantee the total <br />sum OP2,961.00 for the 12 month period <br />beginning October 1, 1979 <br />b) Contingent upon Grantee's performance of services <br />and submission of timely reports required by Rule 9C-2 F.A.C., <br />said funds shall be paid by the Department in equal quarterly <br />amounts. <br />Page 1 of 3 <br />eviscd 8/" <br />OCT 101979 PACE 71 <br />
