BOARD OF COUNTY COMMISSIONERS <br />�r <br />z <br />y <br />r � <br />Month Day, 2010 VIA OVERNIGHT DELIVERY <br />Persons Name <br />Vendor name <br />Vendor address <br />City/State, Zip <br />NOTICE OF AWARD <br />Reference: Indian River County Bid # 2010038 <br />Project Name: Bridge Replacement at Old Dixie Hwy. over the South Relief Canal with Roadway and <br />Drainage Improvements. <br />Dear Mr. , <br />I am pleased to inform you that on , 2010, the Board of County Commissioners awarded the <br />above referenced project to your company. The following documents are required before the applicable County <br />department can issue a "Notice to Proceed" letter. <br />1. Public Construction Bond in the amount of 100% of the contract amount. ($XXXX.00) <br />2. Certificate of Insurance, must name Indian River County as an additional insured and must provide for <br />a 30 day Notice of Cancellation. <br />3. Agreement, execute the enclosed two (2) copies. <br />In accordance with section 255.05(1)(a), Florida Statutes, you are required to execute a Public Construction <br />Bond for the above referenced project. Please submit the Bond and the Certificate of Insurance to this office at <br />the address provided below no later than Date XX, 2010. Failure to comply with the established deadline for <br />submittal of required documents may be grounds for cancellation of the award. <br />Enclosed are two copies of the Agreement.
