Copy of contract or service agreement to be utilized <br />for individual customers which shall include all fees, charges, <br />rentals, late payment charges, cancellation fees, maintenance <br />charges and company rules pertaining to relations between the <br />licensee applicant and subscribers. <br />8. Locations of antenna and transmitter headend facili- <br />ties. <br />9. Certificates of Liability Insurance Coverage in the <br />minimum amounts specified by this Ordinance, naming the County as <br />a co-insured. <br />10. Performance bond as required by Section 10 (c) of <br />this Ordinance, payable to the County. <br />11. City, County, and State Occupational Licenses. <br />12. A statement that details the technical qualifica- <br />tions and experience of the applicant, its employees or agents. <br />13. Such other information the Utility Department may <br />request in order to adequately assess the applicant's qualifica- <br />tions. <br />D. The application shall be accompanied by a non- <br />refundable fee of $1,000.00 and shall be submitted with the <br />completed application to the County Utility Department for review <br />and presentation to the Commission. <br />E. The application shall be reviewed by the County <br />Utility Department and shall be submitted to the Board of County <br />Commissioners upon certification by the Utility Department that <br />the application is in compliance with the Ordinance. Considera- <br />tion of the application by the County Commission shall be at <br />public hearing; notice of such shall be published by the Clerk of <br />the County Commission in a newspaper of general circulation in the <br />County once weekly for two consecutive weeks immediately prior to <br />the week of the date of public hearing; the cost of said Legal <br />Advertisement being borne by the Licensee as an additional cost. <br />F. In the event any application for a license is filed, <br />the area of which overlaps the area of any existing licensee, the <br />existing licensee shall be given notice thereof, together with a <br />dM <br />
