Modular, 1893 90th Avenue <br />Design and Preconstruction <br />Emergency Services District <br />AMENDED WORK ORDER NO. 2 <br />FOR <br />DESIGN/BUILD AGREEMENT <br />PHASE IV <br />WITH <br />Barth Construction, Inc <br />This Work Order No 2 is in accordance with the existing DESIGN/BUILD AGREEMENT <br />PHASE I dated December 6, 2012 between Barth Construction, Inc (Design/Builder) and Indian <br />River County (County) <br />SECTION I — PROJECT LIMITS <br />This Work Order No 2 outlines for the Design/Builder to perform in connection with design, and <br />preconstruction work and related design for a temporary modular building <br />SECTION II - SCOPE OF SERVICES <br />As agreed upon between Design/Builder and County, that the Design/Builder shall provide <br />services to include, <br />• Constructing the electrical, water, fire line and sewer utility connections to the modular <br />building <br />• Constructing the onsite storm water drainage system. <br />• Fill and grade the site including the new stabilized drive that includes 6" of asphalt <br />millings <br />• Installing the concrete parking, including the pavement markings <br />• Erosion control, sod and maintenance during construction and clean up <br />The proposal from the Design/Builder is attached to this work order and incorporated herein <br />1 <br />
