ASSISTANCE PROJECT SCHEDULE CHECKLIST <br />OCTOBER 2015 - Project Agreement Executed, Project Initiates. <br />DECEMBER 30, 2015 - First Quarterly Report Due. <br />MARCH 15, 2016 - Second Quarterly Report Due. <br />JUNE 30, 2016 - Third Quarterly Report Due. <br />SEPTEMBER 15, 2016 - Fourth Quarterly Report Due. <br />DECEMBER 30, 2016 - Fifth Quarterly Report Due. <br />MARCH 15, 2017 - Sixth Quarterly Report Due. <br />JUNE 30, 2017 - Seventh Quarterly Report Due. <br />NOTE: If the project will not be completed and all close out paperwork <br />submitted by September Isr, a request for a 1 -year extension of the completion <br />date of the project should be submitted by July 15, 2017. <br />SEPTEMBER 01, 2017 - Closeout paperwork due. <br />Closeout paperwork must consist of: <br />• Project Completion Certificate, FIND Form No. 90-13 (effective date 7-30-02), which certifies that the <br />project was completed in accordance with the project agreement and the fmal project plans. <br />• A fmal reimbursement request accompanied by all required billing statements and vouchers. <br />• Photograph(s) showing the installation of the sign required by Rule 66B-2.013, F.A.C. <br />• Photograph(s) of the completed project clearly showing the program improvements. (jpg or of format) <br />• A Final Project Report (1-2 pages) that shall at minimum include: project name and address, project <br />summary, fmal cost, and project benefits to the waterway. <br />SEPTEMBER 30, 2017 - District finishes processing closeout paperwork, performs <br />project inspection and submits final reimbursement check with check presentation to <br />sponsor. <br />Forms may be found online on the FIND website: AICW.org <br />NOTE: ANY MODIFICATIONS to the PROJECT shall require advance notice and prior <br />written approval of the District. The appropriate timing for modifications to the project cost <br />estimate, Exhibit A, would be after receipt of bids. <br />*NON-COMPLIANCE by the PROJECT SPONSOR with the reporting schedule in may <br />result in revocation of this agreement pursuant to Paragraph 13 of the project agreement. <br />
