Attachment A—SCOPE OF WORK <br /> Project Name: Indian River County Shooting Range FWC Contract No. 15120 <br /> Hunter Education Classroom <br /> B. COST SHARE <br /> The Grantee agrees to provide 25% of the total cost of the project. The total compensation by <br /> the Commission shall be $144,000.00 or 75%of the total cost for the project, whichever is <br /> less. <br /> C. INVOICE SCHEDULE <br /> The County will submit one request for reimbursement within 30 days after completion of the <br /> Project, as described herein, and acceptance of deliverables in writing by the Commission's <br /> Contract Manager. The Commission shall have 10 working days to inspect and approve <br /> goods and services. <br /> D. TRAVEL EXPENSES <br /> No travel expenses are authorized under the terms of this agreement. <br /> E. FORMS AND DOCUMENTATION <br /> The County shall be reimbursed on a cost reimbursement basis in accordance with Cost <br /> Reimbursement Contract Payment Requirements, attached hereto and made a part hereof as <br /> Attachment D. <br /> The request for reimbursement shall include an invoice in a format similar to Attachment E, <br /> Sample Invoice Form, which shall include the FWC Contract Number,the Grantee's Federal <br /> Employer Identification(FEID)Number, and indicate the dates of service. The invoice shall <br /> be accompanied by a Certification of Completion, photographs to document project <br /> completion, an itemized list of all project expenditures, and copies of invoices and cancelled <br /> checks or check numbers to document payment for all project expenditures. <br /> 6. MONITORING SCHEDULE <br /> A. Compliance Monitoring and Corrective Actions: The Commission will monitor the <br /> County's service delivery to determine if the County has achieved the required level of <br /> performance. If the Commission at its sole discretion determines that the County failed to <br /> meet any of the Terms and Conditions of this Agreement, the County will be sent a formal <br /> written notice. The County shall correct all identified deficiencies within forty-five (45)days <br /> of notice. Failure to meet 100% compliance with all of the Terms and Conditions of this <br /> Agreement or failure to correct the deficiencies identified in the notice within the time frame <br /> specified may result in delays in payment or termination of this Agreement in accordance <br /> with the Termination section. <br /> B. Site Inspections: The Commission may inspect the Project site prior to and, if applicable, <br /> during the construction of the Project. The County shall notify the Commission's Contract <br /> Manager when the Project has reached substantial completion so that inspection may occur in <br /> Version 1.0—June 2013 Page 3 of 5 <br />
