This work was <br />completed in accordance with the Continuing Contract Agreement for Professional Services <br />between Indian River County (IRC) Solid Waste Disposal District (SWDD) and CDM Smith Inc. (CDM <br />Smith), dated December 6, 2011 and extended on November 4th, 2014, under Work Order No. <br />CCNA2014-WO No.1 dated February 17, 2015. This project completion report is to serve as <br />additional information to the final invoice (invoice No. 80542129/11). <br />IRC SWDD is required to perform environmental compliance monitoring and reporting for water <br />and air quality in accordance with the IRC Landfill's operating permits. During calendar year 2015, <br />IRC SWDD was required to submit semi-annual reports of groundwater and surface water quality at <br />the IRC Landfill to the Florida Department of Environmental Protection (FDEP) in accordance with <br />Chapter 62-701.510, F.A.C. and the Monitoring Plan Implementation Schedule (MPIS) Permit Nos. <br />SC31-0128769-019 and S031-0128769-020 as modified by Florida Department of Environmental <br />Protection (FDEP) correspondence regarding the permit renewal application for the Construction <br />and Debris landfill at the IRC facility entitled "Second Request for Additional Information, Permit <br />Renewal Application S031-0128769-021" dated July 5, 2012 and the response by CDM Smith on <br />behalf of IRC dated July 16, 2012. The requirement for leachate sampling and analyses was <br />removed when Chapter 62-701, F.A.C. was revised in August 2012. <br />The IRC Landfill operates under a Title V Air Operation Permit (Permit No. 0610015 -004 -AV), <br />which was issued on July 16, 2012, that contains monitoring and reporting requirements related to <br />the landfill gas collection and control system. IRC SWDD is required to prepare and submit the <br />following to FDEP and the U.S. Environmental Protection Agency (USEPA), Region 4: <br />Ira <br />WATER + ENVIRONMENT +TRANSPORTATION + ENERGY + FACILITIE105 <br />
