C. The OWNER will be given thirty (30) days notice prior to <br /> cancellation or modification of any stipulated insurance. Such <br /> notification will be in writing by registered mail, return receipt <br /> requested and addressed to the OWNER's Risk Manager. <br /> d. An appropriate "Indemnification" clause shall be made a provision <br /> of the Contract (see paragraph 6.20 of the General Conditions). <br /> e. It is the responsibility of the CONTRACTOR to insure that all <br /> subcontractors comply with all insurance requirements. <br /> f. It should be remembered that these are minimum requirements, <br /> which are subject to modification in response to high hazard <br /> operation. <br /> g. Insured must be authorized to do business and have an agent for <br /> service of process in Florida and have a Best's Rating of A-VII or <br /> better. <br /> D. Additional Insureds: <br /> 1. In addition to "Indian River County, Florida," the following individuals or <br /> entities shall be listed as "additional insureds" on the CONTRACTOR's <br /> liability insurance policies: <br /> a. NONE <br /> b. <br /> C. <br /> SC-5.05 OWNER's Liability Insurance <br /> SC-5.05 Delete paragraph GC-5.05.A in its entirety. <br /> SC-5.06 Property Insurance <br /> SC-5.06 Delete paragraphs GC-5.06.A, B, and C in their entirety and insert the following <br /> paragraphs in their place: <br /> A. CONTRACTOR shall purchase and maintain property insurance upon the Work at the <br /> Site in the amount of the full replacement cost thereof. This insurance shall: <br /> 1. include the interests of OWNER, CONTRACTOR, Subcontractors, ENGINEER, <br /> ENGINEER's Consultants and any other individuals or entities identified in the <br /> Supplementary Conditions, and the officers, directors, partners, employees, agents <br /> and other consultants and subcontractors of any of them each of whom is deemed <br /> to have an insurable interest and shall be listed as an insured or additional insured; <br /> 2. be written on a Builder's Risk "All Risk" or open peril or special causes of loss <br /> policy form that shall at least include insurance for physical loss and damage to the <br /> Work, temporary buildings, falsework, and materials and equipment in transit and <br /> shall insure against at least the following perils or causes of loss: fire, lightning, <br /> extended coverage, theft, vandalism and malicious mischief, earthquake, collapse, <br /> debris removal, demolition occasioned by enforcement of Laws and Regulations, <br /> water damage, and such other perils or causes of loss as may be specifically <br /> required by the Supplementary Conditions. <br /> 3. include expenses incurred in the repair or replacement of any insured property <br /> (including but not limited to fees and charges of engineers and architects); <br /> 00800-4 <br /> 00800-Supplementary Conditions LAP rev.doc <br /> 00800 4F\Public Works\ENGINEERING DIVISION PROJECTS\08458-Old Dixie Hwy Sidewalk(38th Ln-45th St)Vldmim\bid documents\LAP Documents\00800- <br /> Supplementary Conditions LAP rev.doc <br />
