06040/2,0 is" <br />• <br />1w•tom' g <br />4. <br />Board of County Commissioners <br />1801 27th Street <br />Vero Beach, Florida 32960 <br />Telephone: (772) 567-8000 FAX: 772-778-9391 <br />Project: <br />WORK ORDER NO. 1 (Engineering) <br />FOR <br />PROFESSIONAL SERVICES AGREEMENT <br />FOR PROFESSIONAL CIVIL ENGINEERING SERVICES CONTRACT <br />WITH <br />Masteller & Moler, Inc. <br />49TH STREET / US1 INTERSECTION IMPROVEMENTS — <br />IRC Project No. 1412 <br />This Work Order No 1 is in accordance with the existing AGREEMENT dated November 15, <br />2011 as amended November 4th, 2014 between Masteller & Moler, Inc (ENGINEER) and Indian <br />River County (COUNTY) This Work Order No 1 amends the agreement as follows <br />SECTION I - PROJECT LIMITS <br />This Work Order No 1 is for the ENGINEER to perform professional engineering services in <br />connection with the planning and design of 49th Street & US1 Turn Lane Improvements at the <br />following locations: <br />1. Eastbound Left -Turn Lane on 49th Street at Old Dixie Highway; <br />2 Eastbound Left -Turn Lane on 49th Street at U. S. Highway# 1; <br />3 Northbound Left -Turn Lane on Old Dixie Highway at 49th Street; and <br />4. Southbound Left -Turn Lane on Old Dixie Highway at 49th Street. <br />The 49th Street Eastbound Left Turn Lane at the approach to old Dixie Highway will require the <br />paving within the Florida East Coast railroad (FECRR) right-of-way to be widened. It is <br />understood the Florida East Coast Railroad will perform all work required for installation of the <br />49th Street Eastbound Left -Turn Lane within the limits of their right-of-way It is anticipated the <br />FECRR may request.a median to be introduced into the turn lane improvements design to allow <br />for calming devices they plan to construct to support the "All Aboard Florida" Railroad <br />improvements. <br />SECTION II - SCOPE OF SERVICES <br />As agreed upon between ENGINEER and the COUNTY, the ENGINEER shall provide <br />Professional Engineering services to complete all tasks as outlined in this Work Order No 1, <br />and per the attached Scope of Services and Cost Outline (Exhibit A) <br />SECTION III - TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT <br />1. Project shall be completed as follows: <br />• Within 70 working days of Notice to Proceed excluding the time expended during <br />the FECRR review of the Conceptual Design Plans <br />
