1 <br />Board of County Commissioners <br />Administration - Building A <br />1801 27th Street <br />Vero Beach, Florida 32960 <br />Telephone: (772) 567-8000 FAX: 772-778-9391 <br />Project: <br />Indian River Boulevard Sidewalk — (37th Street to 53rd Street) <br />Topographic Design Survey <br />IRC Project No. 1415 <br />WORK ORDER NO. 2 (Engineering/Surveying) <br />FOR <br />PROFESSIONAL SERVICES AGREEMENT <br />ANNUAL SURVEYING and MAPPING SERVICES <br />WITH <br />Kimley-Horn and Associates, Inc <br />In accordance with Contract No 1333 2014-2015 <br />This Work Order No. 2 is in accordance with the existing AGREEMENT dated October 1, 2013, <br />and renewed on October 17, 2014 between Kimley-Horn and Associates, Inc , (SURVEYOR) <br />and Indian River County (COUNTY): <br />SECTION I — PROJECT LIMITS <br />This Work Order No 2 is for the SURVEYOR to perform all related field and office surveying <br />and mapping services in connection with the preparation of a topographic design survey for <br />sidewalk construction over a portion of Indian River Boulevard right-of-way. The survey will <br />begin at the intersection of 37th Street and Indian River Boulevard and extend northerly along <br />Indian River Boulevard to the intersection of Indian River Boulevard and 53rd Street. <br />SECTION II - SCOPE OF SERVICES <br />As agreed upon between SURVEYOR and COUNTY, the SURVEYOR shall provide <br />Professional services to complete all tasks as outlined in this Work Order No 2, specifically <br />detailed in the attached proposal Exhibit A. <br />SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT <br />1. Project shall be completed as follows: <br />a 100% "Paper" review submittal (final review prior to request for final deliverables) <br />shall be made within 45 calendar days of receipt of Notice to Proceed for review by <br />County Staff prior to preparing the final submittal package. <br />b Time of FINAL project completion shall be within 10 calendar days of receipt of the <br />County's review comments from the 100% "Paper Submittal". <br />C.\Users\chris.demeter\AppData\Local\Microsoft\Windows\Temporary Internet Files\Content.Outlook\62YG17EF\IRC Kimhey Horn WO No 2 <br />04-09-15.doc <br />
