Davis, P.E.( <br />Public Works Director <br />SUBJECT: Indian River Boulevard south <br />Extension - Change Order #4 and #5 <br />REFERENCES: <br />DESCRIPTION AND CONDITIONS <br />Kimley-Horn and Associates has transmitted Change Order #4 and #5 for <br />approval. Change Order #4 provides for revising roadway superelevation at <br />the south end of Indian River Boulevard. The staff requested this change <br />for the following reasons: <br />1) If the road is widened to six lanes in the future, reducing the <br />superelevation at this time would save considerable cost by reducing <br />the future fill requirement and will also allow for greater drainage <br />capacity in the roadside swales. <br />2) A few citizens have observed the construction and have expressed that <br />the curve looks too banked. <br />3) After the road was designed, Walmart was constructed along the north <br />of the curve. The filling of the Walmart site has substantially <br />raised the elevation along the curve and transition grades are <br />excessive. <br />Change Order #5 is prepared to provide for increasing the size of the steel <br />water and sewer sleaves beneath the Boulevard for the Utilities Dept. This <br />item was previously approved by the Board on Jan. 20, 1987. Also, minor <br />drainage work and concrete driveway work is included. <br />Change Order #4 includes increasing the Time of Completion by 21 days. <br />ALTMUkTIVES AND ANALYSIS <br />The net cost of Change Order #4 and Change Order #5 will be an increase in <br />the contract price by $20,000 and $30,520 respectively. Of this amount, <br />$18,990 will be funded by the Utilities Dept. Impact Fee account. The <br />remainder will be funded by Gas Tax and Impact Fee Revenue. <br />The Change Orders are necessary to complete the work. <br />12ECOMMENDATIONS AND FUNDING <br />It is recommended that Change Order #4 and #5 be approved. Funding to be <br />as follows: <br />Utility Dept. Impact Fees $18,990 <br />Gas Tax & Road Impact Fees $31,530 <br />ON MOTION by Commissioner Bird, SECONDED by Com- <br />missioner Eggert, the Board unanimously approved <br />Change Order #4 (Kimley-Horn/Dickerson) increasing <br />10 <br />
