BOOK � f'�,E l P� <br />FF - <br />APR 7 1987 <br />ATTAC11ME ]T TO <br />CHANGE ORDER NO. 5 <br />FOR <br />12th STREET TRAFFIC & PEDESTRIAN IMPROVEMENTS <br />WITH INDIAN RIVER BOULEVARD, SOUTHERLY EXTENSION <br />EXPLANATION AND COST <br />1. During construction of Indian River Boulevard, the County Public Works <br />Department determined that 10' - 6" diameter PVC drainage pipes beneath <br />the masonry wall along Vista Gardens Condominiums were to be installed <br />to prevent storm water from being trapped behind the wall. <br />1 Job Lump Sum $2,000.00 <br />2. During construction of Indian River Boulevard the Indian River County <br />Engineering Operations Manager revised the schedule of steel casings <br />required for future expansion of the County's utilities systems. <br />1 Job Lump Sum $18,990.00 <br />3. During construction of Indian River Boulevard Kimley-Horn & Associates <br />(KHA) instructed the contractor to install a 24" driveway pipe between <br />Station 222+00 and 222+40 rather than between Station 222+60 and 223+00 <br />as shown on the plans. The installation of the pipe in a different <br />location than shown on the plans was in an effort to better suit <br />conditions found at the construction site. After the pipe and endwalls <br />were installed the County informed KHA that the location of the pipe was <br />to serve a future driveway for the adjacent property owners as shown on <br />the plans. As a result, the pipe will be relocated to the stations <br />shown in the plans. <br />Relocate 24" CMP $20/LF $ 800.00 <br />Remove & relocate endwalls 3 cy @ $250/cy 750.00 <br />$ 1$50.00 <br />4. During construction along 6th Avenue it was determined that the cost of <br />construction of 6" concrete driveways had been omitted from the summary <br />of Estimate Quantities shown in the plans. The cost of concrete shall <br />include 10/10 wire mesh and control joints. <br />6" concrete pavement 360 SY @ $15.50/SY $ 50580.00 <br />Driveway surveying, grading & cutting out of 16 driveways <br />@ $150.00/each $ 21400.00 <br />Total increase per this change order: <br />14 <br />$30,520.00 <br />
