Tia i rman <br />ATTACHMENT I <br />SMTEMENT OF A <br />The Indian River County Health Unit and the Indian River County Board of County <br />Commissioners agree to the following special conditions as they relate to the <br />p <br />Provision of primary care services and the implementation of their contract with <br />the Department of Health and Rehabilitative Services. <br />1. To give priority in service delivery to persons with family incomes below <br />100 percent of the most current federal non-farm poverty guidelines, and <br />to follow the department's eligibility criteria; <br />2• To serve persons below 100 <br />percent of the most current federal non-farm <br />Poverty guidelines at no charge; <br />3. To establish a sliding fee schedule for determining the level of payment <br />for clients with family incomes between 100 and 200 percent of the most <br />current federal non-farm poverty guidelines if the project plans to serve <br />Persons in this income category. Such a fee schedule shallr <br />Percent increments between 100 and 200 percent; P ogress in 20 <br />4. To register all primary care clients in the department's client <br />registration system and to issue a "primary care participant" card to all <br />registered clients; <br />5. TO provide client specific and aggregate reports as required by the <br />deft; <br />6. To follow departmental accounting procedures with respect to the funds <br />awarded in the contract,with the department; <br />7. To develop a consoli <br />sistent with guidedated or unified record for primary care clients con <br />lines established by the department; <br />8. To participate in the evaluation of the primary care project under the <br />department's direction as specified in 154.011(2), F.S.; <br />9. To ensure that subcontractors in the primary care project abide by the <br />sane conditions as those placed on the provider. <br />10. The Indian River County Board of Commissioners will provide the sum of <br />$84,000 as its contribution to the development and implementation of the <br />unary Care Program, <br />d D.
