BARTON <br />ASSISTANT DIRECTOR OF UTILITY SERVICE <br />SUBJECT: CHANGE ORDER 42 & FINAL PAYMENT <br />12TH ST. & COMMERCE AVENUE IMPROVEMENTS <br />INDIAN RIVER COUNTY PROJECT NO. UW -86 -11 -DS <br />BACKGROUND <br />Indian River County Project #UW -86 -11 -DS - 12TH STREET & COMMERCE <br />AVENUE IMPROVEMENTS - was begun by the County as part of the 12TH <br />Street Improvements. The project was designed and bid, with Coastline <br />Utilities being the low bidder, with a contract price of $292,292.00, <br />after your approval of Change Order #1. <br />Upon completion of the project when quantity extensions are reviewed <br />as to materials used on the project, a change order is necessary to <br />adjust the contract price. <br />Attached is Change order #2 for a decrease in the contract price of <br />$22,470.95. <br />The new adjusted contract price is $269,821.05 and the Project Funding <br />is through Account #471-000-169-024.00. <br />RECOMMENDATION <br />Staff recommends to the Board.>of.County Commissioners the following: <br />1. Authorize Chairman to sign Change Order #2. <br />2. Authorize staff to pay final payment request. <br />3. Authorize staff to release retainage upon the completion <br />of the punch list items and upon the receipt of a 1 year: <br />maintenance bond. <br />ON MOTION by Commissioner Eggert, SECONDED by Com- <br />missioner Bowman, the Board unanimously approved <br />Change Order #2 (Coastline Utilities/Masteller & <br />Moler) reducing the contract as set out above; <br />authorized payment of the final payment request; <br />and authorized release of retainage upon completion <br />of the punch list items and receipt of a 1 -year <br />maintenance bond. <br />4 <br />
