APR 11 1989 <br />BOOK 76 PAGE 573 <br />ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Bowman, the Board unanimously (4-0, <br />Commissioner Scurlock being temporarily out of <br />the Chambers) approved the amendment to the IRMH <br />Certificate of Public Convenience and Necessity, as <br />recommended by staff in the above memo. <br />.70%Olt <br />CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY <br />Indian River <br />WHEREAS, the Memorial Hospital'" has requested authorization to provide <br />Interfacility Advanced Life Support services to the citizens of Indian River <br />County; and .. <br />WHEREAS, -there has been demonstrated there is a need to provide•these <br />essential services to the citizens of this county; and, •, <br />WHEREAS, the above named service affirms that it will maintain compliance <br />with the requirements of the Emergency Medical Services Act (Chapter 401, F.S.) <br />and rules (Chapter 10D-66,'F.A.C.), including Indian River County -Ordinance <br />#87-66 as amended. <br />THEREFORE, the Board of County Commissioners -of Indian River County <br />hereby issues a certificate of Public Convenience and Necessity to said • <br />Company to provide Interfacilitv ALS Transport services with <br />(BLS,ALS-transport,ALS non -transport) • <br />limitations as prescribed on this certificate. <br />In issuing this certificate, the governing body of Indian River ' County has Y <br />recommendations of affected municipalities. <br />Date Issued January 10, 1989 <br />(Amended April 11 1989) <br />Limitations: Service is limited <br />Date of expiration January 9, 1991 <br />(unless certificate is sooner revoked or <br />suspended) <br />to interfacility advanced life support and <br />57 <br />
