Release Form <br />The Board reviewed the following memo dated 5/26/89: <br />INDIAN RIVER COUNTY, FLORIDA <br />MEMORANDUM <br />TO: JIM CHANDLER, COUNTY ADMINISTRATOR <br />FROM: STARR JORDAN, RISK MANAGEMENT 4 <br />DATE: MAY 26, 1989 v <br />SUBJECT: RELEASE FORM FROM SEWELL, TODD & BROXTON TO BE PLACED ON <br />THE JUNE 6TH AGENDA <br />Please sign the attached release form to be placed on the June 6, <br />1989 consent agenda. <br />This involved an accident which occurred on 9-8-88 wherein our <br />vehicle was totalled by a Ryder truck. <br />Our insurance carrier at the time, Florida League of Cities has <br />paid the amount of $7828.50 due for a new vehicle. They have <br />already picked up the salvage and all that remains of the old <br />truck. <br />The release form acquits and discharges Ryder Truck Rental, Marie <br />Dorothy Craft and Matthew Thomas Craft from any further claims or <br />actions. <br />If you require any further information please give me a call. <br />ON MOTION by Commissioner Scurlock, SECONDED by <br />Commissioner Eggert, the Board unanimously authorized <br />the Chairman to sign the Release of All Claims, <br />acquitting and discharging Ryder Truck Rental, Marie <br />Dorothy Craft and Matthew Thomas Craft from any further <br />claims or actions. <br />COPY OF RELEASE IS ON FILE IN THE OFFICE OF THE CLERK TO THE <br />BOARD <br />2 <br />
