T. <br />Civil Engineer <br />SUBJECT: Reconciliation Change Order and Final Pay <br />Request from Dennis L. Smith for <br />Miscellaneous Intersections, Phase II <br />DATE: July 20, 1989 <br />----------------------------------------------------------------- <br />DESCRIPTION AND CONDITIONS <br />The construction of the Miscellaneous Intersections, Phase II has <br />been completed by Dennis L. Smith, Inc. The project has been <br />accepted by the County Engineer. <br />The final contract price has been decreased by $14,814.03. This <br />is due to underruns of quantities in the original contract. <br />The contractor is requesting final payment in the amount of <br />.$5,519.62. <br />RECOMMENDATION AND FUNDING <br />Staff recommends approval of the change order #5 to reconcile the <br />contract and final payment to Dennis L. Smith, Inc., in the <br />amount of $5,519.62. Funding is from Fund 101-156-541-067.21 <br />Zone 6 Traffic Impact Fees. <br />ON MOTION by Commissioner Scurlock, SECONDED by Com- <br />missioner Eggert, the Board unanimously (4-0) approved <br />Change Order #5 to reconcile the contract and authorized <br />final payment to Dennis L. Smith, Inc., in the amount of <br />$5,519.62. <br />69 <br />
