In the case of corporations, acceptance must be by an officer of <br />that corporation. Type or print the name and title of the person signing below the <br />signature and the date signed. This indicates that the applicant accepts the. provisions <br />and conditions of the permit. <br />SIGN AND RETURN BOTH THE ORIGINAL AND THE COPY TO THIS OFFICE. <br />The original will be signed by the District Engineer and returned to you with a <br />placard to be posted at the site.
