Exhibit #1 <br />Amendment 2 to <br />Work Order Number 3 <br />North and South RO Plant Lime Slurry Addition <br />Additional Construction Phase Services <br />PROJECT DESCRIPTION <br />The contractor RJ Sullivan is nearing completion of the Lime Slurry Addition Project for both <br />water plants. The original bid included a total contract duration of 180 days with an addendum <br />which provided the contractor an additional 60 days (240 days total). In addition to the addendum, <br />additional work has been authorized which extends their contract 62 days, including painting of the <br />North Clearwell, installation of a new acid tank at the South plant, etc. With the additional work <br />and addendum, the total construction duration is now 302 days. Work Order #3 included <br />construction observation of 5 — 6 months (180 days) which was consistent with the original bid <br />document. Additional construction phase services are therefore needed to cover the additional time <br />of 122 days for field observations, meetings, certifications, pay application review, additional <br />record drawing review, and additional startup services. <br />SCOPE OF SERVICES — CONSTRUCTION PHASE SERVICES <br />Task 1— Degasifier Platform Drawings <br />Consultant will prepare detail drawing sketches of a platform support between the two existing <br />North plant West degasifier platform landings to allow access to the new control valves. The <br />existing platforms do not provide access to the existing valves and are needed for maintenance and <br />access to the new valve actuators. <br />Task 2 — Construction Phase Services <br />The Consultant will provide additional construction phase services for this project, consisting of <br />paint shop drawing review, meeting attendance, construction field observation, pay application <br />review, additional certification, and preparation of record drawings based on contractor red lines <br />(in electronic format in AutoCADD 2010 version in real world coordinates on CD). <br />It is anticipated that the duration of the remaining construction phase will be two (2) additional <br />months and that on-site construction observations will be conducted commensurate with the level <br />of construction activity for each project. <br />Shop drawing submittal will be reviewed for conformance with the intent of the contract <br />documents. Shop drawing will be submitted directly to the Consultant. The Consultant will review <br />the submittal and return copies to the contractor and Indian River County Utilities. <br />The Consultant will coordinate and attend three (3) project status meetings for coordination and <br />status updates of the project with the Contractor and Owner. The Consultant will also review pay <br />requests and provide recommendations for payment to Indian River County Utilities for the <br />progress of the work.
