Nixon, P.E., Roadway Production Manager4/ <br />SUBJECT: <br />DATE: <br />Change Order No. 4 and Release of Retainage <br />Old Dixie Highway Sidewalk from 8th Street to SR 60 <br />IRC Project No. 1102 Bid No. 2012052 <br />March 20, 2014 <br />DESCRIPTION AND CONDITIONS <br />On November 13, 2012, the Board of County Commissioners awarded Bid No. 2012052 to Timothy <br />Rose Contracting, Inc. in the amount of $325,518.37 for the construction of 1''A miles of 5 -foot <br />wide concrete sidewalk along the east side Old Dixie Highway. This project is funded by a Florida <br />Department of Transportation Local Agency Program (LAP) grant, FM No. 425710-1-58-01. Three <br />Change Orders have been approved to date by the Board of County Commissioners, increasing the <br />total contract to $422,321.94. <br />Change Order No. 4 is to make final adjustments to contract bid items to decrease the total <br />contract price by $6,604.14 for a final cost of $415,717.80 and to add justified time due to <br />additional work requested by the County. <br />Timothy Rose Contracting, Inc. has been paid $391,529.62 to date, with $19,576.48 held in <br />retainage. Timothy Rose Contracting, Inc. has completed the project and submitted Pay <br />Application No. 9 (Final) for final payment and release of retainage in the amount of $43,764.66. <br />FUNDING <br />Funding for this project is reimbursable from the FDOT Local Agency Program (LAP) grant, FM No. <br />425710-1-58-01. Payment for Timothy Rose Contracting, Inc.
