INDIAN RIVER COUNTY <br /> CONTINUING CONTRACT AGREEMENT FOR <br /> PROFESSIONAL SERVICES <br /> AMENDMENT #1 TO WORK AUTHORIZATION NO. 6 TO THE CONTINUING <br /> CONTRACT, AGREEMENT FOR PROFESSIONAL SERVICES BETWEEN KIMLEY- <br /> HORN AND ASSOCIATES, INC.AND INDIAN RIVER COUNTY,FLORIDA. <br /> This is an amendment to the existing Continuing Contract Agreement (AGREEMENT) dated <br /> November 15'h, 2011 between Kimley-Horn and Associates, Inc. (ENGINEER) and Indian River <br /> County (COUNTY). This work authorization addresses changes in "Scope of Services" and <br /> "Compensation" of the AGREEMENT. <br /> Amendment #1 Work Authorization Description <br /> This Work Authorization includes the following: <br /> "SCOPE OF SERVICES" is being modified to incorporate the following: <br /> low A. Revisions to Gifford Neighborhood 45`h Street Streetscape Design Plans <br /> ENGINEER will provide professional services as specifically stated below: <br /> I. Addition of Bike Lanes, Curbing of Street_Intersections & H-3,Canal Culvert <br /> Extensions at Street Intersections—Based on our meeting with County staff April <br /> 24, 2014, the COUNTY requested that the ENGINEER revise the landscape <br /> construction plans to include 5-ft paved shoulders along both sides of 45`h Street <br /> between 43rd Avenue and US Highway 1, provide for a westbound right turn lane <br /> at 28`h Court and provide for clear zone mitigation measures at the northern side <br /> street connections along 45`h Street by adding curbing. <br /> In addition, the ENGINEER was requested to delineate future improvements to <br /> the southern side street connections to 45`h Street consisting of side drain <br /> replacements within Indian River Farms Water Control District Lateral H-3 canal <br /> to extend the canal culverts +/- 50' on either side of the cross streets to allow <br /> more landscape opportunity. <br /> ATTACHMENT 2 <br /> 64 <br />
