INDIAN RIVER COUNTY <br />CONTINUING CONTRACT AGREEMENT FOR <br />PROFESSIONAL SERVICES <br />WORK ORDER NO. 8 TO THE CONTINUING CONTRACT AGREEMENT FOR <br />PROFESSIONAL SERVICES BETWEEN KIMLEY-HORN AND ASSOCIATES, INC. <br />AND INDIAN RIVER COUNTY, FLORIDA. <br />This is an amendment to the existing Continuing Contract Agreement (AGREEMENT) dated <br />November 15th, 2011 between Kimley-Horn and Associates, Inc. (ENGINEER) and Indian <br />River County (COUNTY). This work order addresses changes in "Scope of Services" and <br />"Compensation" of the AGREEMENT. <br />Work Order Description <br />This Work Order includes the following: <br />"SCOPE OF SERVICES" is being modified to incorporate the following: <br />Aviation Boulevard/ 20th Avenue Intersection <br />The COUNTY desires to construct a westbound left turn lane at the Aviation Boulevard/ <br />20th Avenue intersection. In addition, the COUNTY desires to signalize the intersection. <br />The signalization improvements shall be constructed to be compatible with the planned <br />ultimate widening and approved plans associated with the Aviation Boulevard Widening <br />Project. <br />A. Roadway Plans: <br />The final design plans will be prepared in accordance with the Indian River County <br />and Florida Department of Transportation (FDOT) standards to provide all elements <br />necessary to accommodate the scope of work. This effort will include at a minimum; <br />Key Sheet, Typical Section Sheet, General Notes Sheet, Plan and Profile Sheets, <br />Cross Section Sheets, Signalization Plan Sheets, Erosion Control Sheets, Opinion of <br />Probable Construction Cost and other detail sheets as necessary to produce the <br />construction bid plans. <br />The design will incorporate the following: <br />1. Roadway plan and profile sheets depicting the roadway improvements. <br />2. Signing and Pavement Marking plans shall include: Preparation of the <br />plan layout. All plans are to be prepared in accordance with the latest <br />design standards and practices identified by the Federal Highway <br />Administration (MUTCD), COUNTY, FDOT Standard Specifications, <br />11Page <br />79 <br />
