Kafer, Jr., P.E., County Engine; <br />SUBJECT: Change Order No. 1 and Release of Retainage <br />12th Street and 27th Avenue Intersection Improvements <br />IRC Project No. 9334 Bid No. 2013021 <br />DATE: December 29, 2014 <br />DESCRIPTION AND CONDITIONS <br />On July 16, 2013, the Indian River County Board of Commissioners awarded Bid No. 2013021 <br />in the amount of $1,525,216.32 to Timothy Rose Contracting, Inc. for improvements to the <br />12th Street and 27th Avenue intersection consisting of the milling and resurfacing of 121h <br />Street approximately 1200 LF east and west of 27th Avenue and the addition of left turn <br />lanes, eastbound and westbound on 12th Street at 27th Avenue. The improvements also <br />include filling the canal along the south side of 12th Street with 60" RCP, as well as curbing, <br />sidewalks, new traffic signal and pavement markings. <br />The project has been completed and Change Order No. 1 is to make final adjustments to bid line <br />items to decrease the total contract price by $100,799.05 for a final contract price of <br />$1,424,417.27 and to add time to contract for delays caused by utility conflicts, weather and <br />design modifications. <br />Timothy Rose Contracting, Inc. has been paid $1,353,196.41 to date, with $71,220.86 held in <br />retainage. Timothy Rose Contracting, Inc. has submitted Contractor's Pay Application No. 6 in <br />the amount of $71,220.86 for release of retainage. <br />FUNDING <br />Payment for Timothy Rose Contracting, Inc.
