IRC Work Order No. 1 - Exhibit 'A" <br />49th Street/ US1 Intersection Improvements <br />Page 4 of 5 <br />Items to Be Provided By Client: <br />To support our design and permitting services on this project, it is understood the County will <br />provide the following information - <br />Right -of -way & Topographic Survey on NAVD 1988 datum <br />Soils Exploration Report <br />Environmental Assessment <br />Wetlands Mitigation Permitting (If Required) <br />Sketch & Legal Descriptions for Right -of -Way Acquisition <br />Additional Services: <br />Our scope of services does not include land surveying, soils investigations, FDOT U.S. Highway <br /># 1 roadway alteration designs, right-of-way acquisition negotiations, landscape / irrigation <br />designs, bidding services, construction administration services. Any services not specifically <br />provided for in the above scope will be considered additional services and can be performed at <br />our contract hourly rates. <br />Deliverables: <br />Deliverables for this project will consist of the following: <br />Conceptual Design 2 Sets <br />4W Preliminary Construction Plans 2 Sets <br />Final Construction Plans 10 Sets <br />Permit Application Forms As Required <br />op", <br />Schedule <br />Upon receipt of the Notice to Proceed and the information to be provided by the County, we <br />shall complete the scope of services for this project on the following schedule: <br />Task Working Days <br />Conceptual Plans 15 Days <br />FECRR Review & Comments Unknown <br />Preliminary Construction Plans 40 Days <br />Final Construction Plans 10 Days <br />Permit Application Forms 5 Days <br />Closure <br />In addition to the matters set forth herein, our Agreement shall include and be subject to, and <br />only to, the terms and conditions in the existing AGREEMENT dated November 15, 2011 and <br />amended on November 4, 2014 between Masteller & Moler, Inc. (ENGINEER) and Indian River <br />County (COUNTY). <br />Fees and times stated in this Work Order are valid for sixty (60) days after the date of this letter. <br />MR 5 <br />
