Change orders #1 & #2 included proposal requests <br />1,2,3,5,6. <br />The following Proposal Request has been issued by the Architect <br />Dow, Howell, Gilmore for changes in the work. <br />Proposal Request #4 - Amount $4,259.00. This relates to <br />changes required by revisions to the.Uniform Fire Safety <br />Standards Code for elevators. These revisions were <br />issued subsequent to bidding and permitting of this <br />project. <br />ALTERNATIVES AND ANALYSIS: <br />Proposal Request #4 has been reviewed by the Architect/Engineer, <br />Construction Manager and Project Manager and is recommended for <br />issuance of Change Order #3. <br />Amount of Change Order #3 will be $49259.00 (Add) with no extension <br />•in time. <br />ON MOTION by Commissioner Wheeler, SECONDED by <br />Commissioner Bird, the Board unanimously (4-0, <br />Commissioner Scurlock being absent) approved the <br />issuance of Change Order #3 for an additional $4,259 <br />with no extension in time, as recommended by staff. <br />CHANGE ORDER #3 IS ON FILE IN THE OFFICE OF THE CLERK TO THE BOARD <br />REQUEST FOR ADDITION OF .12TH COURT S.W.
