NOV 2 0 1990 <br />BOOK 81 PAGE 926 <br />Proposal Request_.#23 - Additional wall framing under window frames <br />J & K - amount $125.07. This request is for additional work <br />required to support interior window frames modified in Proposal <br />Request #9 (previously approved). <br />Proposal Request #24 - Ground fault interrupters - amount $204.60. <br />This change was required by the City of Sebastian Electrical <br />Inspector and provide ground fault protection for floor -mounted <br />receptacles in the children's workroom. <br />Proposal Request #25 - Fire zone map - amount $139.15. This change <br />was required by the South County Fire Inspector and provides a map <br />of all smoke detector locations within the building. <br />.Proposal Request #26 - Deletion of the column bases (exterior) - <br />amount ($315.00). This change was requested by Project Manager for <br />maintenance purposes. <br />Proposal Request #27 -Addition of door closers - amount $445.04. <br />This change is required to add four (4) door closers on fire rated <br />doors. This item was not shown on the hardware schedule but is <br />required by the building code. <br />Proposal Request #28 - Additional electrical circuits - amount <br />$301.51. The plans did not clearly show two (2) electrical <br />circuits. These circuits were not included in the panel board <br />schedules. <br />RECOMMENDATION AND FUNDING <br />All work outlined above has been completed by the contractor. <br />Proposal Requests #14, 16, 17, 19, 21, 22, thru 28 were granted <br />field authorization by the Construction Manager with concurrence of <br />this writer to ensure prompt completion of the project.
