DEPARTMENT OF GENERAL SERVICES <br />,SUBTECT:''INDIAN RIVER COUNTY HEALTH UNIT <br />CHANGE ORDER NUMBER FIVE <br />.':BACKGROUND: <br />`;The following Proposal Requests are hereby submitted for the subject <br />project.which will necessitate a change order to the contract with <br />,Miorelli Engineering and Indian River County. <br />P.R.'35 - Moved walls and enlarged door openings to accommodate <br />specified doors. Cost of $139.13 and no additional time. <br />P.R. 39 -'Item 1. Changed doors.in restrooms to match door <br />':schedule. Cost of $2 09(2{00 and no additional time. <br />:•;:; Item 2. Extended concrete over infiltration ditch in <br />parking lot. Cost of $797.00 and no additional time. <br />P.R. 40 - Changed light fixtures in two rooms to accommodate concrete <br />slab. Cost of $110.83 and no additional time. <br />P.R. 41 - Redig and install storm water drain pipe section. Cost of <br />$236.00 and no additional time. <br />ANALYSIS• <br />Net effect on the original contract would be an increase of $3,375.00 <br />or a total contract price of $2,444,906 and no additional days. <br />Completion date remains at May 10, 1991. <br />RECOMMENDATIONS: <br />Staff recommends Board approval of Change Order as submitted and <br />requests authorization for the Chairman to execute the documents. <br />r <br />ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Scurlock, the Board unanimously <br />approved Change Order #5 with Miorelli Engineering <br />for the Public Health Unit and authorized the <br />Chairman to execute the documents as recommended by <br />Staff. <br />SAID DOCUMENT IS ON FILE IN THE OFFICE OF <br />CLERK TO THE BOARD <br />20 <br />
