Baird, County Administrator <br />From: Vincent Burke, P.E., Director of Utility Services <br />Prepared By: Arjuna Weragoda, P.E., Capital Projects Manager Aw <br />Gordon Sparks, P.E., Environmental Engineer <br />Subject: 66th Avenue Phase 2 Utility Conflict Resolutions, <br />Final Pay to Carter Assoc., Inc., for Work Order No. 2 — UCP 4040 <br />DESCRIPTIONS AND CONDITIONS: <br />On February 9, 2010 the Board of County Commissioners (BCC) approved Continuing Consulting <br />Services Work Order No. 2 with Carter Assoc., Inc. for $24,700.00 survey, design and construction <br />services for 66th Avenue Phase 2. The purpose of Work Order No. 2 was to resolve conflicts <br />between existing utilities and proposed roadway drainage and paving improvements. <br />ANALYSIS: <br />The roadway improvements have now been completed and the conflicts were successfully resolved <br />during construction. Carter Assoc., Inc. submitted Application for Payment No. 14 Final on January <br />30, 2014 for $3,304.46. Previous payments to date total $21,155.11 resulting in a final total cost for <br />the consulting services of $24,459.57, or $240.43 less than budgeted. <br />FUNDING: <br />Funds for this project are derived from the other professional services account in the operating <br />fund.
