Attachment D <br /> Reports <br /> The following reports must be completed and submitted to the Department in the time frame <br /> indicated. Failure to timely file these reports constitutes an event of default, as defined in <br /> Paragraph(10) of this Agreement. <br /> 1. The Contractual Obligation and MBE Report must be submitted to the Department by <br /> April 15 and October 15 annually. The form must reflect all contractual activity for the <br /> period. If no activity has taken place during the reporting period, the form must indicate <br /> "no activity". <br /> 2. A Monthly Progress Report must be submitted to the Department fifteen (15) days after <br /> the end of the month on the report form provided by the Department. The report is due <br /> on the 15`h of the following month. <br /> 3. The Administrative Closeout Package must be submitted to the Department forty-five <br /> (45)days after the Agreement termination date. <br /> 4. In accordance with OMB Circular A-133, revised, should the Recipient meet the <br /> threshold for submission of a single or program specific audit, the audit must be <br /> conducted in accordance with OMB Circular A-133 and submitted to the Department no <br /> later than nine(9)months from the end of the Recipient's fiscal year. <br /> 5. The Section 3 Summary Report must be completed and submitted to the Department by <br /> July 31 annually. The form must be used to report annual accomplishments regarding <br /> employment and other economic opportunities provided to persons and businesses that <br /> meet Section 3 requirements. <br /> 19 <br />
